Case Management
Planning and service coordination to help prevent
out of home placement and more restrictive services
Case Management Services for children:
- Assess the strengths and needs of the child and caregivers;
- Provides a link between the caregiver and school personnel to coordinate and monitor services;
- Advocates on behalf of child at school, medical, FAPT, and IEP meetings;
- Explores and identifies community resources to benefit child and family;
- Educate both child and family regarding appropriate behavior and coping strategies;
- Assists in the transition between Intensive In-Home Outreach Services and Outpatient Therapy Services.
Referrals may be made by parents or guardians. If a child is already receiving outpatient therapy services from the Community Services Board, the child’s therapist will assist with making the referral. If a child is not already receiving outpatient therapy services from CSB, call one of the Counseling Centers and ask to speak with an Emergency Services Counselor:

Fees for services are covered under most Medicaid plans; private payment may be arranged on an individual basis, including the use of a sliding fee scale.
