Case Management

Most people need occasional help with organizing and managing their lives – fitting together needs for housing, food, clothing, employment, health care, income, and recreation.  Individuals with mental retardation have the same needs and may also require help with planning and coordinating their total living needs.

Case Management is a comprehensive service that assesses an individual’s strengths and needs for services.  A case manager is assigned to each person (adult or child) who requests mental retardation services in order to coordinate resources and facilitate independence and stability.  Services include assessment, planning, linking, and monitoring services, as well as assisting families by advocating on their behalf, identifying needs, and developing individual service plans.

Case Management is also involved with pre-admission screening (only after all community resources have been considered) and pre-discharge planning for individuals referred for community living from an institution.
