Case Management

To support independent living, we assess, counsel, and refer individuals to services and programs that meet their specific needs as  identified on their comprehensive treatment plan. We coordinate services and  treatment planning in cooperation with other agencies and providers and then monitor  the quality of service by keeping in touch with service providers and making  periodic visits to home and program sites.
